Lasting Power of Attorney (LPA)

Lasting Power of Attorney (LPA) is a legal document which allows a person who is at least 21 years old (“donor”) to voluntarily appoint one or more persons (“donee(s)”) to make decisions and act on the donor’s behalf should he or she lose mental capacity one day.

A medical doctor or a registered psychiatrist can issue a certificate to certify that the donor understands the implications of making an LPA.

Advance Medical Directive (AMD)

An Advance Medical Directive (AMD) is a legal document that you sign in advance to inform the doctor treating you (in the event you become terminally ill and unconscious) that you do not want any extraordinary life-sustaining treatment to be used to prolong your life. Making an AMD is a voluntary decision.

How can I make a Lasting Power of Attorney?
1. Access the OPG Online (OPGO) Portal: Log in using your Singpass to complete the LPA form.
2. Appoint Donee(s): Specify one or more trusted individuals (donees) to make decisions on your behalf if you lose mental capacity.
3. Certification: Book an appointment with our doctors to certify your LPA. Our doctor will ensure you understand the LPA and are not under duress.
4. Submission: The LPA certificate will be submitted online via the OPGO portal.
• Application Fee: The application fee for LPA Form 1 is currently waived for Singapore Citizens until 31 March 2026.
• Clinic Certification Fee: Our clinic charges $50 (before GST) for LPA certification by our doctor.

2. Complete the Form: Visit our clinic to complete the form in the presence of our doctor, who will confirm that you understand the AMD and are of sound mind.
3. Submission: Our clinic will submit the completed AMD form directly to the Registrar of AMDs at the Ministry of Health on your behalf.

You can make an AMD at our clinic. Our doctor will guide you through the process, certify the form, and submit it to the Ministry of Health.

• AMD: States your wish not to receive extraordinary life-sustaining treatment if you are terminally ill and death is imminent. It only applies to end-of-life medical decisions.
• LPA: Appoints one or more donees to make decisions on your behalf if you lose mental capacity. It covers broader decisions, including personal welfare, finances, and medical care.
